Our educational booklet “Breathe” was originally designed as a compliment for our patients as they transition to home from their post-acute stay with easy to understand language about co-morbidities, disease process and empowerment. This comprehensive booklet now serves as a conduit for all facets of a patient’s journey throughout healthcare and is used by home health, clinics & physicians.

Root Cause Analysis Report

All Patients in our disease management programs, who are readmitted to the hospital within 30 days, may have a root cause analysis (RCA) report performed. This is provided free of cost to your facility. The RCA is broken down into three different areas: Evaluation, conclusion and resolution. Nursing progress notes, RT notes, vitals, Physician orders, X-rays and lab work are all examined during a root cause. This enables us to identify the many possible causes for an effect, or problem, during the patient admission. The resolution gives your facility ideas and solutions to help prevent future hospitalizations. It also ensures accountability within your facility and educates your healthcare providers.
